This form, submitted by Colonel William K. Bailey, commander of the 311 Air Commando Squadron, verifies that all records and sources have been considered and formally recommends Lieutenant Colonel Joe M. Jackson for the Medal of Honor. Jackson “distinguished himself by gallantry and intrepidity in action, at risk of life, above and beyond the call of duty at Kham Duc, Republic of Vietnam on 12 May 1968.”
